OTA Students - Annual policy can only be WAIVED in the FALL. FALL WAIVER MUST BE COMPLETED by September 21, 2018.

Only New incoming Spring students can waive for Spring by February 8th, 2019.

* International students and students in the A.S. Occupational Therapy Assistant program students are required to have health insurance coverage. The policy will be billed automatically for these students. Students who have private coverage may waive this policy by clicking here.

** The University highly recommends the needlestick policy coverage for Health Science students. This policy provides insurance coverage should a student be exposed to blood-borne pathogens. Students may opt-in by contacting the Student Accounts office.
